Continuity Care Home Nurses provides nursing and other therapeutic services to patients of all ages in the comfort of their own homes. Home health care needs ranging from highly technical nursing to personal assistance are met by our professional, highly qualified, experienced and carefully screened staff. Care in the home usually results in a shorter overall recuperation period for acute illness. For the patient with chronic active diseases, the home setting provides a desirable alternative to extended hospital stays or to skilled nursing facility placement.
At a time when increased demands are being made on an overburdened and costly health care delivery system, we believe that increased utilization of the home as a therapeutic environment may assist this problem and that notable savings can be realized. Patients are happier, more comfortable and more closely attended when they receive health care services in their own homes. The patients really appreciate the opportunity to preserve much of their independence by remaining at home and to be able to participate in their health care management since they are in familiar surroundings. The home care experience is very satisfying and rewarding, not only for the patient, but also for the patient’s family and patient care team. There is also quicker recovery and less risk that the patient is exposed to nosocomial and secondary infections as they would be in the hospital environment.
Licensed nursing services are provided according to an individualized treatment plan under the direction of the patient’s physician and supervised by our director of nursing. We will also provide support for other health professionals in the home through our multi-disciplinary approach- including physical therapists, occupational therapists, speech therapy, dietary consultation, medical social workers, and certified home health aides as ordered by the patient’s doctor.
- Continuity Care Home Nurses provides a warm, supportive, and caring environment for both the patient and family members. We will advise our patients in setting up their home for the most convenient and supportive environment for patient care, including advising them on the need for specific items, i.e. Hospital bed, bedside commode, walker, and other items. We work in the patient’s home under the patient’s physician’s orders and work closely with the physician to provide the total care necessary for the patient.
- We provide a preadmission patient assessment and a free insurance evaluation at no cost or obligation to the patient. Home care may be covered under the patient’s insurance plan. Upon referral to our agency, the patient’s insurance coverage will be verified by our knowledgeable staff. Our director of nursing will find out what the patients needs are and based on knowledge of the medical necessity requirements, will help the patient determine exactly what is covered under their insurance plan.
- Continuity Care Home Nurses is very concerned about the quality of care we provide. All patients are sent one of our “patient satisfaction surveys" in the mail shortly after their care begins. All surveys are reviewed personally by our director of nursing and appropriate follow up is done.
